How the GAD-7 is scored
Each of the seven items is rated 0 (Not at all) to 3 (Nearly every day) based on how often it bothered you over the last two weeks. Item scores are summed for a total between 0 and 21.
- 0 – 4 — Minimal anxiety
- 5 – 9 — Mild anxiety
- 10 – 14 — Moderate anxiety
- 15 – 21 — Severe anxiety
A cutoff score of 10 is the standard threshold suggesting further evaluation. At this cutoff the GAD-7 has 89% sensitivity and 82% specificity for identifying generalized anxiety disorder (Spitzer et al., 2006).
